Our intake specialists speak English and Spanish. If you decide to move to the next step after the initial conversation, your representative will visit you in your home, interview you about your needs and expectations and answer any questions you or your caregiver(s) might have.
PACE is ideal for those on Medi-Cal or Medi-Cal/Medicare combo. In fact, if you have Medi-Cal with no share of cost, there is no out of pocket cost for PACE services. If you have a Medi-Cal share of cost, it is paid monthly to St. Paul’s PACE.
St. Paul’s PACE also accepts private pay.
TAKE A TOUR
We will coordinate a tour of the St. Paul’s PACE Center near you.
You will see and experience many services that St. Paul’s PACE provides.
You will meet other participants and members of our staff.
Transportation and lunch will be provided if you wish.
ASSESSMENT
After your tour, you will be invited to meet with members of our care team including our Registered Nurse and Social Worker. You may also meet with a Physical Therapist, Occupational Therapist, Doctor and other members of the team.
Transportation and lunch will be provided.
The care team will develop a Level of Care Report on your health conditions and send it to the State to get approval to enroll you in PACE. This is the approval for Nursing Home Level of Care as determined by the Department of Health Care Services’ Long-Term Care Division.
ENROLLMENT CONFERENCE
If you are approved and you wish to enroll, you will receive your Member Enrollment Agreement Terms and Conditions and sign your Enrollment Agreement.
The Care Team members will explain major features of the program such as:
- Your monthly fee, if any.
- That St. Paul’s PACE does not pay for medical care you receive outside the plan without prior authorization, except for emergency services.
- That you use the St. Paul’s PACE Primary Care Physician.
- What to do if you are unhappy with the care you receive at St. Paul’s PACE (You will be provided with Participant Rights, Grievance and Appeal Policies and Procedures).
- How to disenroll at any time for any reason (You will be provided with Disenrollment Policy and Procedures).
START WITH PACE
Your enrollment in St. Paul's PACE is effective the first day of the calendar month following the date you sign the “Enrollment Agreement.”
Eligibility Criteria
- Adults 55 years of age or older.
- Living in the San Diego County designated approved zip code service area.
- Determined by the Department of Health Care Services for nursing home level of care [PACE coordinates this determination for you].
- Able to live in a community setting without jeopardizing health or safety.
