How To Qualify For Elderly Services

Enrollment in St. Paul’s PACE is voluntary and individuals qualify if they are:

• Adults 55 years of age or older
• Living in the designated service area (see our map and zip code listings)
• Determined by the Department of Health Care Services for nursing home level of care [PACE coordinates this determination]
• Able to live in a community setting without jeopardizing their health or safety

PACE is ideal for those on Medi-cal or Medi-cal/Medicare combo. We welcome you to contact our enrollment center at the phone number below or e-mail us

PACE elderly services are supported and regulated by the Centers for Medicare & Medicaid Services and the California Department of Health Care Services.

St. Paul’s PACE accepts Medi-Cal or Medi-Cal/Medicare combo. If you are entitled to Medi-Cal, you pay no premium, if you do not have Medi-Cal; contact us to determine your premium.
