San Diego assisted living services are offered at both at our Elm Street Center and in the seniors' home. Our Participants are picked up from their home and driven to St. Paul’s PACE where they receive San Diego assisted living services specific to their individual care plan such as bathing, social recreation, doctor’s appointments, physical therapy and more. Therefore, we can at this time only offer San Diego assisted living services to seniors in our service area (zipcodes in the sidebar). We are looking to expand PACE and will hopefully be able to work in a larger amount of San Diego County soon.
