I want to stay in my home, but I can't afford in home nursing service. How do I enroll with St. Paul’s PACE?

St. Paul’s PACE makes the enrollment process easy. Our enrollment specialist will meet with you (and Caregivers/Family members if desired) at your home to provide a detailed understanding of the St. Paul’s PACE program and services. They will also do an assessment of eligibility during the visit.

We will then invite you to to take a tour of one of our senior citizen centers on Elm street, and of course we will provide the transportation. While at PACE, you will also meet with our Registered Nurse for a brief medical assessment and enjoy lunch before we take you home.

We then send your nursing assessment to the DHCS LTC Division (Department of Health Care Services, Long Term Care Division) where your application is approved. Once we receive the approval, you can start with PACE the following month.
