10 Years with St. Paul’s PACE: Irma’s Story of Care, Independence and Community
For 10 years, Irma has been part of the St. Paul’s PACE community. During that time, she has received support to help her live safely at home while enjoying time at our centers and building friendships with other participants. Her story highlights how coordinated healthcare and supportive services can help older adults maintain their independence while staying connected to their communities.
St. Paul’s PACE, or the Program of All-Inclusive Care for the Elderly, provides comprehensive medical care and support services to eligible older adults throughout San Diego County. The program is designed to help participants meet their healthcare needs while continuing to live in their own homes and communities whenever possible.
Building Friendships and Staying Connected
Healthcare is an important part of healthy aging, but opportunities for social connection also play a role in daily life. St. Paul’s PACE centers provide participants with places to spend time with others, participate in activities and build friendships.
Participants can enjoy social activities, connect with staff and develop relationships with other older adults who share their experiences. These connections can help make time at the centers an important part of their routines.
Irma’s 10 years with St. Paul’s PACE reflect both the support she receives to help her live safely at home and the community she has become part of along the way.
The program combines coordinated healthcare with opportunities for engagement and connection, allowing eligible participants to receive services while remaining involved in their communities.
A Decade of Being Part of the St. Paul’s PACE Community
Every participant has a different story. For some, PACE helps them manage complex medical needs. For others, services such as transportation, rehabilitation or in-home assistance can make everyday life more manageable. Many participants also value the opportunity to spend time at the centers and maintain social connections.
Irma’s experience is an opportunity to recognize the role that coordinated care and community can play in supporting older adults. Her 10 years with St. Paul’s PACE represent a decade of being part of a program focused on helping eligible seniors access the services they need while continuing to live at home.
