How it Works

As a St. Paul’s PACE participant, you will receive tailored medical care from a team of health professionals who specialize in aging and chronic conditions. Our doctors, nurse practitioners, and registered nurses work with therapists, dietitians, social workers, and home care nurses to create a customized plan that is right for you!

All of your health care - including specialized services - is provided by St. Paul’s PACE or a contracted provider authorized by St. Paul’s PACE.

A Team of Healthcare Professionals

With St. Paul’s PACE, a team of healthcare professionals works with both you and your family to ensure you receive the coordinated care that you need. They’ll even get to know you on an individual level, offering additional services that aim to support your overall well-being. In addition to your primary care physician, we have many specialists who can provide you with expert medical attention in a variety of fields, including:

Audiologist: Hearing specialist
Dentist: Care of your teeth
Optometrist: Vision specialist
Podiatrist: Care for your feet
Psychologist: Support for mental health

If you need to see a specialist, all appointments are handled through your PACE primary care physician and approved by your care team. This coordination ensures that you receive the best possible medical treatment and that all necessary parties are properly informed about your health needs.

St. Paul’s PACE offers virtual telehealth, appointments to help minimize exposure for participants who are immunocompromised. If you have a medical emergency outside of regular business hours, our nurses can be available to support you 24/7.

With St. Paul’s PACE, you’ll have all the help you need in one place. The best part? All services are provided at no cost* to you if you have Medi-Cal or a combination of Medi-Cal and Medicare. Even if you do not currently have Medi-Cal, one of our Enrollment Specialists can help determine if you might be eligible. If your health team deems you need care and services that Medicare and Medi-Cal do not cover, St. Paul’s PACE may still cover them.

*Those with a Medi-Cal share of cost remain responsible for payment.
