What is PACE
… and who is it for?
St. Paul’s Program of All-Inclusive Care for the Elderly (PACE) is a managed health care plan exclusively for seniors. As part of St. Paul’s PACE, you receive primary medical care, medication management, physical therapy, specialty services, in-home care, social work assistance, and transportation to and from the medical center. With St. Paul’s PACE, you can remain living in the comfort of your home and get the medical care you need in a safe environment.
St. Paul’s PACE is specifically designed for seniors 55 years and older who have chronic medical conditions and are struggling to live at home independently.
Unlike most medical programs, St. Paul’s PACE provides in-home care services. Upon your initial assessment, a home care nurse will evaluate your needs to determine if home care services will help you to continue living safely at home.EXPLORE In HOME CARE

The St. Paul’s Difference
St. Paul’s PACE has cared for more than 1,500 San Diego seniors since 2008, helping them age safely and successfully in their homes. The PACE model was first introduced in San Diego by St. Paul’s Senior Services, an organization that has served the local senior population since 1960. Since then, St. Paul’s Senior Services has continued to evolve and offer new programs and senior living solutions based on the developing needs of the geriatric population.
