With St. Paul’s PACE, you have a team of medical experts and health care professionals working directly with you and your family to ensure you get the most effective plan of care.

Program of All-inclusive Care for the Elderly

Your healthcare team provides a hands-on approach, coordinating medical, social, and home care services so you no longer need to manage your medical care alone.

You will have a team of qualified, bilingual, caring doctors and staff to provide you with “all-inclusive” medical, social, nutritional, rehabilitation, home care, and meal services. Your healthcare and social service professionals are experts in working with seniors.

Through St. Paul’s PACE you get all the medical care and services covered by Medicare and Medi-Cal, as approved by your team of healthcare professionals, as well as medically-necessary care and services not covered by Medicare and Medi-Cal.
