St. Paul’s PACE provides senior care for adults in San Diego over 55 years of age who are living with chronic illness or disabilities and need coordinated medical care to continue living as independently as possible in their home and community.

About St. Paul's PACE

Our senior healthcare teams provide a hands-on approach, coordinating medical, social, and home care services so our participants no longer need to manage their medical care alone.

We use a team of qualified, bilingual, caring doctors and staff to provide “all-inclusive” medical, social, nutritional, rehabilitation, home care, and meal services. These healthcare and social service professionals are experts in working with seniors. They work together with St. Paul’s PACE participants and their family members to develop the most effective plan of care.

St. Paul’s PACE provides all the medical care and services covered by Medicare and Medi-Cal, as approved by our team of healthcare professionals, as well as medically-necessary care and services not covered by Medicare and Medi-Cal.

All-inclusive care consists of:

  • Primary care (including doctor visits & nursing services)
  • Medical specialty services
  • Prescription drugs
  • Durable medical equipment (DME)
  • Senior day care in our beautiful wellness centers
  • Health education & nutritional counseling
  • Rehabilitation therapy (physical /occupational / speech therapy)
  • Social Services
  • Mental & Behavioral Health Services
  • Recreational Therapy
  • Vision & dental services
  • Podiatry
  • Laboratory / X-ray services
  • Home Care / Personal Care Assistance
  • Meals
  • Transportation
  • Hospital care, as required
  • Nursing home care, as required
  • Emergency services (24 hours per day, 7 days a week)

Do I Qualify?

Is St. Paul’s PACE for you? Are you over 55 years of age and struggling to live at home? Perhaps you are concerned that you may fall or are confused about medication management. St. Paul’s PACE may be ideal for you. St. Paul’s PACE is the medical care program for seniors specifically designed to keep you in your home by providing services in-home and at our wellness centers. We provide all-inclusive medical, social, and home care services, and we coordinate it all so you don’t have to. We even coordinate specialist appointments like dentists and podiatry and provide transportation. To become a St. Paul’s PACE participant, you must be:

55 years of age or older

Live in our service area of San Diego Central, East or South Counties. Note: North county coming soon!

Be able to live at home safely with the aid of PACE services

Be certified by the State of California Department of Health Care Services

This approval process is easy. A St. Paul’s PACE registered nurse will conduct a very simple assessment for you. Please call our friendly bi-lingual enrollment specialists:

PACE Model Of Care

Your St. Paul’s PACE care team members (also known as the Interdisciplinary Team or IDT) is composed of a primary care physician, center manager, registered nurses, physical, occupational and recreational therapists, social worker, dietitian, your home care certified nursing assistant, St. Paul’s PACE center aides, and transportation representatives.

As needed, your care team mobilizes other services including medical specialists, laboratory and other diagnostic tests, and hospital and nursing home care. When you first join St. Paul’s PACE, you will receive an in-depth health assessment from your entire St. Paul’s PACE care team. Additional assessments will happen every six months; when there is a change of condition or when a request for services is made. You and your caregiver(s)/family will continue to play an important role in managing your health and independence.
