How it Works
Unlike most medical programs, St. Paul’s PACE will come to your home to provide a wide variety of services. As part of your initial assessment, our nursing staff will determine if home care would be helpful in supporting your current lifestyle. Depending on your needs, our nurses can help you with bathing, meal preparation, laundry, medication management, light cleaning and more.
All home care is provided by trained and certified professionals who will be monitoring your ongoing medical condition.
Caring & Qualified Nurses
You may have noticed that some of your tasks around the house have grown more difficult as you have aged. While everyone’s capabilities and medical needs are different, it’s quite common for seniors who are otherwise quite independent to encounter stumbling blocks in their home routines.
If you find yourself struggling with certain chores or hygiene tasks, there is no need to panic about losing your independence.
The in-home assistance provided by St. Paul’s PACE can help fill in the gaps in your daily routines and ensure that no parts of your home life will fall through the cracks.

When you decide to proceed with the enrollment process, you’ll be invited to visit one of our four wellness centers for an assessment. Our courteous staff will be happy to answer any questions you have about home care and work with you to schedule a meeting with an Enrollment Specialist in the comfort of your home.
