Regardless of the reason, St. Paul’s PACE includes the option for in-home care, bringing assistance to your front door.

How It Works

Unlike most medical programs, St. Paul’s PACE will come to you to provide a wide variety of services within your home. As part of your initial assessment, our nursing staff will determine if in-home care would be helpful in supporting your current lifestyle. If you do need in-home support, we provide a range of services:

● Bathing
● Meal preparation
● Laundry
● Light cleaning
● Medication management
● And more, depending on your needs.

All home care is provided by trained and certified professionals who will be monitoring your ongoing medical condition, to ensure that we are providing the assistance you need to function well.

You may have noticed that some of your tasks around the house have grown more difficult as you have aged. While everyone’s capabilities and medical needs are different, it’s quite common for senior citizens who are otherwise quite independent to encounter stumbling blocks in their home routines.

If you find yourself struggling with certain chores or hygiene tasks, there is no need to panic about losing your independence. The in-home assistance provided by St. Paul’s PACE can help fill in the gaps in your daily routines and ensure that no parts of your home life will fall through the cracks.

It can be stressful to attempt to care for yourself and your home with no outside help. Our qualified and caring nursing staff can help ease some of your burdens and enable you to enjoy your home life to the fullest.

A visit from one of our Enrollment Specialists will help you decide if St. Paul’s PACE is for you. When decide to proceed with enrollment, we will invite you to one of our three wellness centers for an assessment. Our wellness centers are St. Paul’s PACE Reasner in San Diego, St. Paul’s PACE Akaloa in Chula Vista and St. Paul’s PACE East in El Cajon. You can call any of our centers. Our courteous staff will be happy to answer any questions you might have about in-home care and work with you about scheduling a meeting with our Enrollment Specialist in your home.
