St. Paul’s PACE (Program of All-Inclusive Care for the Elderly) operates four locations across San Diego County, bringing full-service senior care to the communities that are central, Downtown San Diego, Chula Vista, El Cajon, and North County (Encinitas).

What Is St. Paul’s PACE?

Before diving into each location, it helps to understand what PACE actually is. St. Paul’s PACE is a managed healthcare program exclusively for seniors 55 and older who have chronic medical conditions and want to continue living at home rather than move to a nursing facility or assisted living community.

The program covers an extraordinarily wide range of services, primary medical care, physical and occupational therapy, home care, transportation, social work, behavioral health, dental, nutrition support, and day center activities, all coordinated by a single interdisciplinary team. For seniors who qualify through Medi-Cal (or Medi-Cal combined with Medicare), all of these services are provided at no cost.

St. Paul’s PACE has served more than 4,000 San Diego seniors since launching in 2008, making it the longest-running PACE program in the county.

San Diego (Downtown) – 111 Elm Street

The original St. Paul’s PACE location sits in the heart of Downtown San Diego, making it the central hub of the program’s care network. It serves seniors living throughout central San Diego, including surrounding neighborhoods accessible via St. Paul’s door-to-door transportation service.

The Downtown location offers the full range of PACE services, including on-site primary care from physicians who specialize in geriatric medicine, physical and occupational therapy, behavioral health support, social work services, and an active day center. Participants have access to specialists across a wide variety of disciplines, audiologists, optometrists, dentists, podiatrists, and more, all coordinated through one care team.

Chula Vista (South Bay) – 630 L Street

St. Paul’s PACE Chula Vista opened in 2012 in response to a recognized need for medical and social services for lower-income seniors in the South Bay area. The City of Chula Vista welcomed the program as a significant resource for the community, and it has served South Bay seniors ever since.

The Chula Vista location is notable for its bilingual services. Many of the care team members are bilingual practitioners, and translation services are available for participants who speak other languages, a meaningful feature in a community as diverse as South Bay.

In addition to comprehensive medical services, the Chula Vista center offers a large day center with a full schedule of engaging activities: arts and crafts, pet therapy, exercise programs, board games, and more. The goal isn’t just entertainment,  these activities have been shown to improve mobility, memory, coordination, and overall mood over time.

St. Paul’s PACE El Cajon operates as a true one-stop shop for senior healthcare. Participants work with physicians, social workers, dietitians, physical and occupational therapists, and behavioral health. The location also connects participants to specialists as needed, handles prescription management, and provides door-to-door transportation.

The El Cajon day center is one of the things participants frequently point to as life-changing. Arts and crafts, music therapy, pet therapy, puzzles, and regular social events give seniors a reason to get out of the house and connect with peers their own age.

The North County location in Encinitas serves seniors throughout the northern reaches of San Diego County, an area spanning communities like Carlsbad, Oceanside, Vista, San Marcos, Escondido, and surrounding cities.

Telehealth has become an especially important part of the North County offering. When medically appropriate and requested by the participant, physicians conduct appointments by phone or video, giving seniors in more spread-out North County neighborhoods the flexibility to receive care without the burden of a long drive.
