St. Paul’s PACE Continues to Invest in the Future of Care With New EHR Partnership

At St. Paul’s PACE, innovation is part of how we continue to meet the evolving needs of the seniors we serve. As healthcare changes, so do the tools and systems needed to deliver coordinated, efficient care.
St. Paul’s PACE is collaborating with Collabrios Health on an electronic health record (EHR) solution designed specifically for PACE organizations. The partnership will support St. Paul’s PACE in strengthening workflows, improving efficiency and giving care teams the technology they need to stay connected and focused on participants.
Adopting new technology is not simply about keeping up, it is about looking ahead. Collabrios’ PACE platform is built around the unique workflows of PACE, including interdisciplinary care coordination, care planning, analytics and operational efficiency. By identifying opportunities to improve processes and reduce administrative work, teams can spend more time focused on what matters most: coordinating quality care for participants and supporting their ability to remain independent in their homes and communities.
This latest step reflects St. Paul’s PACE’s ongoing commitment to evolving alongside the needs of San Diego’s aging population. The collaboration came together through a connection with Trias, which introduced St. Paul’s PACE to Collabrios. From there, the conversation grew into an opportunity to explore what a more modern EHR could look like for PACE. Since bringing the PACE model to San Diego, St. Paul’s has continued to expand and adapt its services as the needs of older adults change. Partnerships like these help create opportunities to share ideas, explore new solutions and continue building a more connected future for PACE.
