St. Paul’s PACE Celebrates National PACE Month by Honoring Staff and Participants

San Diego, CA (September 2025) — St. Paul’s Program of All-Inclusive Care for the Elderly (PACE) is proud to celebrate National PACE Month by spotlighting the dedicated staff and inspiring participants who make this wraparound medical program thrive. This year’s theme, “Your Health. Your Journey. Our Support.”, reflects St. Paul’s PACE mission to provide personalized care that helps older adults live safely, independently, and with dignity.

At St. Paul’s PACE, the heart of the program lies in the people. Team members like Erin, a skilled medical nurse who ensures participants receive the highest quality care, and Bo, a compassionate social worker who guides participants through their personal journeys with encouragement and advocacy, embody the PACE model’s commitment to holistic support.

Equally inspiring are the participants who call St. Paul’s PACE home. Andrew, a participant who initially needed support with mobility, has made incredible progress thanks to physical therapy, nutritious meals, and encouragement from staff. Today, Andrew enjoys more independence and confidence, a true reflection of how PACE empowers older adults to live life on their terms.

St. Paul’s PACE was the first PACE program in San Diego County, currently serving over 1,400 seniors across its centers. By offering medical care, social services, transportation, meals, and more all coordinated under one program. PACE helps older adults to remain in their homes and communities rather than moving into nursing facilities.

As National PACE Month continues, St. Paul’s invites the community to learn more about how PACE is shaping the future of senior care while honoring the incredible individuals, staff and participants alike who make it possible.

St. Paul’s PACE supports seniors so they can continue to live independently at home by providing personalized medical services, social services, transportation, and home care. Seniors who are 55 years or older with chronic medical conditions that make it difficult to live at home may qualify for services.
